TERMS AND CONDITIONS

CANCELLATION / REFUND POLICY
Cancellation requests must be submitted in writing to info@LoneStarVolleyball.net prior to the close of regular registration. A $35 administrative fee is non-refundable for
accepted cancellations.
Refunds or credits will NOT be issued after regular registration closes, NO exceptions.

LEAGUE TEAM TRANSFERS
For league teams, individual participants are not assigned to teams based on requested practice conflicts nor are they moved from their assigned team to accommodate practice
conflicts. Once a participant is assigned to a team, they will remain on that team’s roster for the current season.

PRIVACY POLICY
FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL) only collects personal information when you request our services and only uses the information collected to
provide those services for you. If you choose to make a purchase, you will be asked to provide contact information (such as your name and shipping address) and financial
information (such as your credit card number and expiration date). This information is used for billing purposes and to process your order. FRISCO ATHLETIC NETWORK, INC.
(DBA LONESTAR VOLLEYBALL) handles all of this data with the utmost confidentiality.

THIRD PARTIES
Any personal information provided will not be traded, rented, sold or otherwise shared with anyone outside of FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL),
or used for reasons beyond those contemplated in this statement. From time to time, we may engage third parties to process your information on our behalf; however, none of
FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL) affiliates (such as our credit card company) have permission to retain, share, store or use personal information
for any reason other than providing the requested service.

SECURITY
To prevent unauthorized access, disclosure and improper use of your information, we have established appropriate physical, electronic, and managerial safeguards to protect
the information we collect in accordance with our privacy policy.

Your payment and personal information is always safe. Our Secure Sockets Layer (SSL) software is the industry standard and among the best software available today for secure
commerce transactions. It encrypts all of your personal information, including credit card number, name, and address, so that it cannot be read over the internet.” (etc.)
Furthermore, your credit card number, expiration date and CSV code is never stored in our systems.

Our systems are routinely scanned by a third party provider for security vulnerabilities in compliance with the policies of VISA / MASTERCARD.

AGREEMENT BETWEEN USER AND FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL)

The network of FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL) operated by FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL) or its affiliates or subsidiaries, is comprised of various Web sites and Web pages. The FRISCO ATHLETIC NETWORK, INC. (DBA LONESTAR VOLLEYBALL) websites are offered to you conditioned on

Facility Cleaning Protocol

LoneStar Sports Center and 9th Island Cafe will follow cleaning procedures posted by the CDC.

  • The water fountain will not be available during this time per State requirement.
  • The Café’s Food Truck will be open during regularly scheduled training times. All Food Safety requirements will be adhered to. Pick up beverages and snacks prior to entering the facility only.
  • Training exercise equipment will be sanitized after each player
  • Volleyballs will be sanitized prior to a practice, during a practice and after a practice with sanitizing wipes

All Participants may be screened at the door

  • Anyone entering the facility may have their temperature checked and asked:
    • Have you come in contact with anyone that is sick or has COVID-19 symptoms?
    • Have you had any of these symptoms within the last 14 days?
    • Fever or chills, Cough
    • Shortness of breath or difficulty breathing
    • Fatigue, Muscle or body aches
    • Headache
    • New loss of taste or smell
    • Sore throat, Congestion or runny nose
    • Nausea or vomiting or Diarrhea
    • Have you come in contact with anyone that is sick or has COVID-19 symptoms?
  • Only 1 parent may enter the facility with their athlete if required for that child.
    • Parents entering the facility must wear a protective face mask at all times while in the facility.

Individual Cleaning Protocol – Protect Yourself and Others

  • Stay home if you are sick or have any new symptoms of possibly being sick.
  • Stay home if you’ve been around anyone who has been ill within the last 14 day whether you show symptoms or not.
  • Do NOT touch any part of your face at any time
  • If you must sneeze or cough, cover your nose/mouth with your arm or bend in inside elbow. Immediately after, wipe down the area with a sanitizing cloth and through it in the trash.
  • If you become ill while in the facility, immediately inform your coach.
  • Do not bring excess items to the gym. Leave personal items in your car.  If a cell phone is required to stay with you, it must be wiped down with sanitizing cloths at the time of entry.
    • Do not touch cell phone during training unless there is an emergency
  • Sanitize hands upon arrival and before going to the court area. Sanitizing stations are on the walls at both the front and back doors, around restrooms and on the wall of every court. 
    • If allergic to alcohol-based sanitizer, wash with soap in the restroom.
    • No more than 3 people in the restroom at any time.
  • Sanitize hands frequently when on the court
    • While on water break
    • While waiting for a turn in line to complete a skill
  • Once training is complete, sanitize hands before leaving the court and the facility.

Parents will:   

  • Ensure that all player registration and payment is completed prior to the start of the program (No walkups)
  • Follow the Entrance, Exit, Drop Off, Pick Up instructions listed under each Program
  • Do not arrive with your player earlier than 15 minutes prior to the start of the scheduled program. See Program Details.
  • In general, parents should not enter the facility. If required, only 1 parent per player may enter the facility, must wear a face mask and follow Individual Cleaning Protocol.
    • Parents are not permitted in the court area at any time. Bleachers are off limits unless otherwise specified under Program Guidelines.  If a parent must stay the duration, they must wait in the seating area at the front of the facility.
    • Parents that must stay are required to follow the age group entrance/exit plan as listed under each Program.
  • Not step onto the court surface at any time for any program.
  • Ensure their player is in the gym on time and no later than 10 minutes after start time. There will be no late arrivals or admittance after this time, no exceptions.
  • Pick up player by the posted end time of the program to pick up their child.
    • We cannot have players picked up late due to the strict capacity limitations, our cleaning time, team/player transitions and our staff duties in between training sessions.
      • A violation may result in the player being suspended from the entire session with no refund or credit for any remaining dates.

Players will:

  • Follow the Entrance, Exit, Drop Off, Pick Up instructions listed under their program
  • Bring their own water bottle
  • Leave all personal items in the car or at home (no backpacks, earrings, cell phones, tablets, etc). If a player must bring in a cell phone, it must be wiped down with a sanitizing wipe when brought into the facility and left on the bleachers at their court.
  • Come to the facility dressed out to practice. Do not bring in extra clothes, etc.
  • Immediately wash/sanitize hands before stepping onto a court or touching any equipment.
  • Be respectful of distancing
  • Not touch another player (high five, huddles, etc.) at any time
  • Frequently sanitize their hands at a court sanitizing station
  • Not gather in close groups with staff or other players
  • Not enter the restroom with more than 3 at a time
  • Report that they are feeling ill immediately to their coach
    • Not leave the facility without reporting that they are feeling ill to their coach or LoneStar staff
  • Not hang out in the gym after training has been completed.
  • May choose to wear a face mask during physical activity, but only during training sessions (i.e. not allowed to be worn by players on the court during matches). 

LoneStar Staff and Volunteer Coaches will:

  • Park in the front parking lot and enter through the Front Door then exit through the Front Side Door.
  • Be checked for temperature and screened for symptom history in their household and workplace upon entering facility.
  • Leave all personal items in the car or at home (no backpacks, earrings, tablets, etc). If a coach must bring in a cell phone, it must be wiped down with a sanitizing wipe when brought into the facility and left in their pocket or identified storage area in the facility.
  • Be present on their court at least 15 minutes prior to practice to greet, gather and monitor players since parents will not be present
  • Bring their own water bottle
  • Bring a change of clothes if they are training more than 1 program back to back
  • Immediately wash/sanitize hands upon entering and frequently throughout training
  • Place cones on the court to remind players of keeping a distance from one another
  • Remind players frequently to sanitize their hands and not to touch any part of their head above the shoulders and not to touch each other at any time.
  • Limit the number of balls used in training
  • Wipe down balls and cart during water breaks
  • Keep players divided on both sides of the court. No more than 6 players (for game drills or games) should be on one side of the court at any time.
  • Separate players when providing group discussions (no huddle) or demonstrating group skills
  • Maintain a safe distance from players and other staff
  • Stagger water breaks from other courts so that we limit the number of players potentially off the court in the restroom or sanitizing station.
  • Not gather in close groups with other staff or players
  • Immediately inform LoneStar management if a coach or player reports they are not feeling well or gets sick while training.
  • Move an ill coach or player to the designated holding area. The parent will be contacted immediately to pick up the child.
  • LoneStar staff will send an immediate announcement to teams practicing of potential contact

PROGRAM GUIDELINES

Academy / Camps & Clinics:  Entrance, Exit, Drop Off, Pick Up

To provide a clean, safe environment for players, maintain proper capacity limits and allow for proper distancing, LoneStar has implemented a strict policy regarding the drop off and pick up of players.  It is imperative that we follow this protocol so that safety precautions are followed, and the facility can be prepared for stacked training times.

  • The FRONT door is to be used only to drop off of players. There will be NO EXIT from the front door other than an emergency situation.
  • The BACK door is to be used to pick up players. There will be NO ENTRANCE from this door other than an emergency situation.
  • Drop Off Players (Players coming into the facility)
    • The FRONT door is to be used only for drop off.
    • Doors will remain locked until check-in has begun.
    • Check-in at the front desk will begin 15 minutes prior to the start of scheduled program.
    • 1st-4th Grades
    • Parent will walk their player to the front check in desk. Inform the staff member which court the player is to go to (See Court Schedule for team’s or Welcome Letter for individual training program’s court assignment prior to entering).  A staff member will walk the player to their court.
    • Parent will exit through the Front Side Door only
    • 5th- Ac. II
      • Player may be dropped off at the front door. A staff member will remain at the front door until 10 minutes after the posted start time.  Players will check in at the front desk.
    • Players will not be admitted after 10 minutes past the posted start time. Staff will be engaged in the program and unable to provide proper check-in requirements after this time.
    • No credit, refund or make-up will be provided for the missed session because of lateness.
  • Pick Up Players (Player picked up after completion of training)
    • All players MUST be picked up from the back door immediately after the posted completion time of the training. The doors will be locked within 10 minutes after the completion of the program so that coaches and staff may properly sanitize before the next group entering.
    • A player may be removed from the program with no refund or credit if picked up after the 10 minute grace period.
  • Normal Coach to Player ratios will be maintained, however, the groups will be divided evenly on 2 sides of the net. This places no more than 6 players on one side of the net.

Club and League Teams – Practices and Games

  • Practices
    • Only if required (preferably only for 1st-4th graders), 1 parent per player may enter the facility for practices and must follow Parent Protocol
    • Enter the facility no more than 10 minutes prior to a practice through the designated door as listed below.
    • Players may move to their court no more than 10 minutes prior to the start of practice.
    • Players should be divided on both sides of the court
    • For skills drills, divide evenly on both sides of the court
    • For game drills, one side should have 6 players on one side with the additional players on the opposite side
  • Games
    • Only 1 parent may enter the facility with their child for games and must follow Parent Protocol
    • Parent may sit on only 1 side of the court and remain there through the duration of the game. Do not change sides.  Maintain social distancing in the bleachers.
    • Enter the facility no more than 15 minutes prior to a practice through the designated door as listed below. Games will not begin before the posted start time and all teams will receive a 5- minute warm up on their designated court.
  • Only 3 courts will be scheduled with games, therefore, we may need to use more Saturdays than normal.
  • Teams will be assigned a warm-up court and will stay on that court until they are called to the game court. Balls for each warmup court will be provided.  Balls will be sanitized prior to the next scheduled match.
  • Teams will not change sides between each set
  • There will be 30 minutes scheduled in between matches to allow for a change in teams and sanitizing equipment.
  • Once a match is over, both teams should exit the building immediately after per the exit plan listed below. Do not gather for team meetings within the facility or in groups larger than 10.
  • Team roster will be reduced to a minimum required of 8 players during a pandemic event
  • The bench area will be marked to keep those waiting to play 6’ apart. A player on the bench may wear a facial mask, if preferred.
  • There will be no player to player handshakes, high fives, etc. before, during or after play
  • There will be no player to player touching for position transitions or substitutions
  • Players must be spaced out arms-length in timeout huddles
  • Due to the heavier traffic created by league practices and game days, we will be assigning entrance and exit doors based on your court assignment. You must use the entry/exit door for your court, so please be sure to park accordingly.  The intent is to limit the group size entering and existing the facility.  Court assignments will be posted on the game schedule once it’s released. 
    • Courts 1/2 Front Door
    • Courts 3/4 Back Side Door (west of Main Back Door)
    • Court 5/6 Main Back door
    • Courts 7 Front Side Door (west of front door)

Private Training

  • Will be scheduled on-line only through our website at LoneStarVolleyball.net > Private Training
  • Each lesson will be scheduled on a specific court. Please check the court assignment on the confirmation email or on our website at LoneStarVolleyball.net > Court Calendar.
  • Payment will be made no later than 1p the day following the lesson using the payment methods listed on the confirmation email.
    • Only optional tips may be handed to the coach in cash at the time of the lesson
  • Parents will drop player off in the front lobby only. Parents are not permitted beyond the front red doors of the facility for Private Training to minimize touched surfaces.
  • Player may arrive no more than 10 minutes prior to the scheduled start time of the lesson and must go directly to the assigned court.

 In consideration of being allowed to participate in any way in the program, related events and activities, I the undersigned, acknowledge, appreciate, and agree that:

I am aware there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof. 

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS Frisco Athletic Network, Inc. dba LoneStar Volleyball, its officers, officials, agents and/or employees, other participants, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Liability Waiver - Release and Indemnity Agreement

RELEASE AND INDEMNITY AGREEMENT

This Release and Indemnity Agreement (“Agreement”) is given by Account Holder, individually (the “Adult Participant”), and as parent or legal
guardian for and on behalf of Registered Participant, a minor (the “Child Participant”; together with the Adult Participant, the “Participant”), to, in favor of
and for the benefit of the Protected Parties as defined by the Release and Indemnity Agreement.

As valid consideration for entry into and participation in the Activities (defined below) with Frisco Athletic Network, Incorporated (“FAN”), including
any of those Activities that may occur in, about, or from the premises located at 10570 John W. Elliott Drive, Suite 600, Frisco, Texas, 75033 (the “Leased
Premises”) or any other premises wherever located, the Participant agrees as follows:

1. Release of Claims. To the fullest extent permitted by law, the Participant (and on behalf of his or her heirs, executors and representatives) hereby
RELEASES Frisco Athletic Network, Inc., which is the tenant of the Leased Premises and any affiliates or subsidiaries of the foregoing, and all of their
respective officers, directors, shareholders, members, managers, partners, agents, employees, contractors, and representatives (collectively, the “Protected
Parties”) from and against all liabilities, losses, damages, claims, demands, actions, suits, costs, fees, and expenses (including reasonable attorney’s fees and
court or other costs) relating to, resulting from, or arising out of or alleged to have arisen out of (in whole or in part) any bodily injury to or death of the
Participant or damage to or loss of the Participant’s property

(a) during or relating to the Participant’s participation, whether actively or passively, in any training, competition, event, or program sponsored by or
affiliated with FAN or any of the other Protected Parties (collectively, the “Activities”), or

(b) occurring in, about, or from any premises (including the Leased Premises) where any of the Activities are taking place, conducted, or performed by the
Participant or anyone else, or in transportation to and from any of the Activities.

THE RELEASE CONTAINED IN THIS PARAGRAPH WILL APPLY EVEN IF ANY SUCH INJURY OR DAMAGE IS CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OR
STRICT LIABILITY OF THE PROTECTED PARTIES OR THE PARTICIPANT, BUT WILL NOT APPLY TO THE EXTENT ANY SUCH INJURY OR DAMAGE IS CAUSED BY THE
WILLFUL MISCONDUCT OF THE PROTECTED PARTIES.

2. Indemnity. The Participant (and on behalf of his or her heirs, executors and representatives) agrees to INDEMNIFY, DEFEND, AND HOLD HARMLESS
the Indemnified Persons from and against all claims, actions, suits, losses, liabilities, damages, fines, penalties, liens, judgments, settlements, proceedings,
costs, fees, and expenses (including reasonable attorney’s fees and court or other costs) of any nature whatsoever for or relating to death, bodily injury or
property damage resulting from, relating to, or caused by (whether in whole or in part) any of the following matters:

(i) the Participant’s acts, omissions or presence on or about any part of the Leased Premises or other premises where Activities are taking place, conducted,
or performed by the Participant or anyone else,

(ii) the Participant’s active or passive participation in, or observance of, any of the Activities; and

(iii) the Participant’s use of any fixtures, equipment or personal property in, on or about Leased Premises or other premises where Activities are taking place,
conducted, or performed by the Participant or anyone else.

THE INDEMNITY CONTAINED IN THIS PARAGRAPH WILL APPLY EVEN IF ANY SUCH INJURY OR DAMAGE IS CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE
OR STRICT LIABILITY OF THE PROTECTED PARTIES OR THE PARTICIPANT, BUT WILL NOT APPLY TO THE EXTENT ANY SUCH INJURY OR DAMAGE IS CAUSED BY
THE WILLFUL MISCONDUCT OF THE PROTECTED PARTIES.

3. Authority. As the parent or legal guardian of the Child Participant, I represent to the Protected Parties that I have legal capacity and authority to act
for and on behalf of the Child Participant, and I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS the Indemnified Persons from and against all claims or
liabilities as a result of or relating to any insufficiency of my legal capacity or authority to act for or on behalf of the Child Participant in the execution of this
Agreement.

4. Acknowledgment and Understanding. The Participant represents to the Protected Parties that he or she thoroughly understands that this is a
complete and final release and indemnity agreement, that he or she is voluntarily entering into this Agreement, and that no representations, promises or
statements made by any of the Protected Parties has influenced the Participant in causing him or her to sign this Agreement. The Participant further agrees
that there are no oral agreements, representations, promises or warranties that are not expressly set forth in this Agreement.

5. Miscellaneous. This Agreement

(i) and all matters arising under or relating to this Agreement, shall be governed by, and construed and interpreted in accordance with, the laws of the State
of Texas;

(ii) constitutes the entire agreement and understanding between the parties with respect to the subject matter hereof and supersedes all prior agreements
(whether written or otherwise) relating to such subject matter;

(iii) is binding not only on the Participant, but also his or her heirs, executors and representatives; and

(iv) will not be construed more or less favorably between the parties by reason of authorship or origin of language.

If a provision in this Agreement is unenforceable for any reason, this Agreement is to be construed as if the unenforceable provision is not a part of the Agreement.
The Adult Participant is acknowledging this Agreement as of the date of registration.

COACHES
• I hereby pledge to provide positive support, care, and encouragement for all players participating in youth sports by following this Coach’s Code of Ethics Pledge.
• I will place the emotional and physical well-being of my players ahead of a personal desire to win.
• I will treat each player as an individual, remembering the large range of emotional and physical development for the same age group.
• I will do my best to provide a safe playing situation for my players.
• I will promise to review and practice the basic first aid principles needed to treat injuries of my players.
• I will do my best to organize practices that are fun and challenging for all my players.
• I will lead by example in demonstrating fair play and sportsmanship to all my players.
• I will provide a sports environment for my team that is free of drugs, tobacco, and alcohol, and I will refrain from their use at all youth sports events.
• I will be knowledgeable in the rules of each sport that I coach set forth by Frisco Athletic Network, Inc., and I will teach these rules to my players and their parents.
• I will use those coaching techniques appropriate for each of the skills that I teach.
• I will remember that I am a youth sports coach, and that the game is for children and not adults.

PARENTS / ALL ADULT PARTICIPANTS
• I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parents’ Code of Ethics Pledge.
• I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice, or other youth sports event.
• I will place the emotional and physical well-being of my child ahead of a personal desire to win.
• I will insist that my child play in a safe and healthy environment.
• I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all.
• I will demand a sports environment for my child that is free of drugs, tobacco, and alcohol, and will refrain from their use at all youth sports events.
• I will remember that the game is for youth-not for adults.
• I will do my very best to make youth sports fun for my child.
• I will ask my child to treat other players, coaches, fans, and officials, with respect regardless of race, sex, creed, or ability.
• I promise to help my child enjoy the youth sports experience by doing whatever I can, such as being a respectable fan, assisting with coaching, or providing transportation.
• I will require that my child’s coach be trained in the responsibilities of being a youth sports coach and that the coach upholds the Coaches’ Code of Conduct.
• I will read the Rules of the Game as set forth by Frisco Athletic Network, Inc. so that I have a better understanding of the game.
• I will immediately notify the Executive Director/Head Referee in the event that I witness any illegal activity.
• I will NOT harass, intimidate or bully teammates, other players or coaches or the officials, including line judges and scorers
• I will NOT bring and/or carry any firearms at any FAN event.
• I will NOT use tobacco products or illegal drugs on the premises.
• I will NOT bring, purchase or consume alcohol at any FAN event.
• I will NOT do physical damage to the facility in which we practice or play or conduct theft of items from the facility or another person

PLAYERS
• I hereby pledge to be positive about my youth sports experience and accept responsibility for my participation by following this Players’ Code of Conduct Pledge.
• I will encourage good sportsmanship from fellow players, coaches, officials and parents at every game and practice by demonstrating good sportsmanship.
• I will attend every practice and game that I can, and will notify my coach if I cannot.
• I will expect to receive a fair and equal amount of playing time based on my attendance at practices and games.
• I will do my very best to listen and learn from my coaches.
• I will treat my coaches, other players, officials and fans with respect regardless of race, sex, creed, or abilities and I will expect to be treated accordingly.
• I deserve to have fun during my sports experience and will alert parents or coaches if it stops being fun!
• I deserve to play in an environment that is free of drugs, tobacco, and alcohol and expect adults to refrain from their use at all youth sports events.
• I will encourage my parents to be involved with my team in some capacity because it’s important to me.
• I will do my very best in school.
• I will remember that participating in sports is an opportunity to learn and have fun.
• I will NOT harass, intimidate or bully teammates, other players or coaches or the officials, including line judges and scorers
